If you are interested in becoming a patient at COC please click on the "new patient" tab and follow the instructions to submit the necessary paperwork.  If you have questions about the services we offer please call us at 614-785-1115, option 1. It is the patient’s responsibility to contact his/her insurance company to verify in-network status for their provider, copay and deductible information. Please note that we do not provide disability assessments, nor will our providers complete paperwork for disability services.

In order to schedule an appointment you will need to click "take me to my forms" below.  Please complete Packets #1 and #2.  These forms are submitable thru our website or you may download, print, complete, fax or email back to us.  Please include a copy of your photo ID & front/back of your insurance card.  The simplest way to do this is to take pictures with a smart phone and email them to us at newpatient@centralohiocounseling.com.  Once we have all the necessary paperwork & card copies our Intake Scheduler will call you to set up an appointment.
