Due to the COVID-19 virus continuing to be a concern in Central Ohio, our office has decided to adhere to the recommendations of the CDC and the Ohio Department of Health and limit face to face contact to prevent the spread of this virus.  We are offering virtual office visits to all patients, including new patients.  At this time we are not offering "in office" appointments.

If you are interested in scheduling a new patient appointment please click the "New Patient" tab at the top of this page and fill out the forms located in this tab, along with emailing us a photo of the front & back of your insurance card and your photo ID.  Once we've received all of these documents we will reach out to you to set up your first appointment.

Existing patients that are in need of a medication refill please be advised that you will need to call our office and schedule a virtual office visit with your provider.
